Grief Support Guide: Feedback Survey

Thank you for taking the time to complete this short survey about the Grief Support Guide. We are gathering people’s views and feedback on this first version of the Guide to help us understand how well it is working and to make any improvements that may be needed in the next version.

Your answers will only be seen by the research team based at Cardiff University. We do not ask for names or contact details in this survey and any information/comments that you provide will be anonymised to make sure you cannot be identified.

By participating in this survey, you are agreeing that we may include your anonymised survey responses in future reports and publications. Anonymised results from the survey may also be presented at conferences, in promotional material (e.g. press releases, on websites) and for teaching purposes.  

All data collected in the survey will be held securely by the survey software provider Jisc Online Survey (www.jisc.ac.uk) and then retained by Cardiff University for 5 years after the survey closes in accordance with Cardiff University policies, General Data Protection Regulations (GDPR; EU 2016/679) and the Data Protection Act 2018 (DPA 2018).
