For the purposes of this questionnaire Cardiff University is the Data Controller and is registered with the Information Commissioner’s Office to process personal data (Reg no Z6549747). The information you provide will be processed by the Counselling, Health and Wellbeing Service in order to provide you with appropriate advice, services and support. All data collected in your submission will be held confidentially and securely by the Software Provider (Bristol University) under contract and normally retained by Cardiff University, in accordance with Records Management Policy and Records Retention Schedules, for a period of 6 years following the end of your relationship with the service. Your personal information will not be made available to anyone outside the Student Support and Wellbeing Service without your consent unless this is necessary to discharge a statutory function (i.e duty of care) or unless required to do so by law. Any disclosures will be in accordance with Data Protection law and your interests will always be considered. Some of the information you provide will be anonymised and may be made available to other parts of the University and more widely for statistical and analysis purposes to help inform and improve the services on offer.
